Patient Rights and Responsibilities

As a patient, you have certain rights and responsibilities. Understanding these helps ensure you receive the best possible care and helps us serve you effectively.

1Your Rights as a Patient

Respectful Care: You have the right to considerate and respectful care regardless of your age, gender, race, national origin, religion, sexual orientation, gender identity, or disabilities.

Information: You have the right to receive complete information about your diagnosis, treatment, and prognosis in terms you can understand.

Informed Consent: You have the right to receive the information necessary to give informed consent before the start of any procedure or treatment.

Refuse Treatment: You have the right to refuse treatment to the extent permitted by law and to be informed of the medical consequences of your refusal.

Privacy: You have the right to every consideration of privacy concerning your own medical care program.

Confidentiality: You have the right to expect that all communications and records pertaining to your care will be treated as confidential.

Continuity of Care: You have the right to expect reasonable continuity of care and to be informed of available and realistic patient care options.

Review Records: You have the right to review the records pertaining to your medical care and to have the information explained, except when restricted by law.

2Your Responsibilities as a Patient

Provide Information: You are responsible for providing accurate and complete information about your health, including past illnesses, hospitalizations, and medications.

Ask Questions: You are responsible for asking questions if you do not understand your treatment plan or what is expected of you.

Follow Treatment Plans: You are responsible for following the treatment plan recommended by your provider or informing them if you cannot comply.

Respect Others: You are responsible for being considerate of other patients and staff and for respecting the property of others.

Meet Financial Obligations: You are responsible for meeting the financial obligations of your healthcare as promptly as possible.

Keep Appointments: You are responsible for keeping appointments and notifying us if you cannot do so.

Communicate Changes: You are responsible for informing your provider of any changes in your condition.
