1How to Request Your Records
Complete a Medical Records Release Authorization form, which can be obtained from our office or downloaded from our website.
Submit the completed form to our Medical Records department by mail, fax, or in person.
Include specific information about what records you need and the date range.
Specify where you want the records sent (to yourself or another provider/entity).
2Processing Time
We will process your request within 30 days of receipt.
If we need additional time, we will notify you and explain the reason for the delay.
Urgent requests may be accommodated when possible. Please indicate if your request is urgent and the reason.
3Fees
There may be a fee associated with copying and mailing your medical records.
Fees are based on state regulations and may include a per-page charge plus postage.
We will inform you of any fees before processing your request.
Records sent electronically may have reduced or no fees.
4What Records Can Be Requested
You may request all or part of your medical record.
Records may include clinical notes, treatment plans, assessments, and correspondence.
Some records may be withheld if your provider determines that disclosure would be harmful.
Psychotherapy notes (if any) may require a separate authorization.
5Transferring Records
To transfer your records to another healthcare provider, please provide their name, address, and fax number on the authorization form.
We can send records electronically to providers with compatible systems.
You may also request records for legal, insurance, or personal purposes.
