Credit Card On File and Financial Responsibility Policy
This policy outlines our financial procedures, including our credit card on file policy, to ensure a smooth and transparent billing process for all patients.
1Credit Card On File Policy
We require all patients to have a valid credit or debit card on file. This card will be used to collect any patient responsibility amounts after insurance processing.
Your card information is stored securely and in compliance with PCI-DSS standards.
We will notify you before charging your card and provide an itemized statement of charges.
You may update your card on file at any time by contacting our billing department.
2Insurance Billing
We will bill your insurance company directly for covered services.
You are responsible for understanding your insurance benefits and coverage.
Co-pays, deductibles, and co-insurance amounts are due at the time of service or when billed.
If your insurance denies a claim, you will be responsible for the full amount of the service.
3Self-Pay Patients
If you do not have insurance or choose not to use your insurance, payment is due at the time of service.
We offer various payment options and may be able to provide information about financing options.
Please discuss self-pay rates with our billing department before your appointment.
4Cancellation and No-Show Policy
We require at least 24 hours notice for appointment cancellations.
Late cancellations (less than 24 hours) and no-shows may result in a fee.
This fee is not covered by insurance and is your responsibility.
Repeated no-shows may result in discharge from the practice.
5Collections
Accounts with balances over 90 days may be sent to collections.
We will make reasonable attempts to contact you before sending an account to collections.
You are responsible for any collection fees or legal costs associated with collecting unpaid balances.
