Continuity of Care: You have the right to expect reasonable continuity of care and to be informed of available and realistic patient care options.
Review Records: You have the right to review the records pertaining to your medical care and to have the information explained, except when restricted by law.
2Your Responsibilities as a Patient
Provide Information: You are responsible for providing accurate and complete information about your health, including past illnesses, hospitalizations, and medications.
Ask Questions: You are responsible for asking questions if you do not understand your treatment plan or what is expected of you.
Follow Treatment Plans: You are responsible for following the treatment plan recommended by your provider or informing them if you cannot comply.
Respect Others: You are responsible for being considerate of other patients and staff and for respecting the property of others.
Meet Financial Obligations: You are responsible for meeting the financial obligations of your healthcare as promptly as possible.
Keep Appointments: You are responsible for keeping appointments and notifying us if you cannot do so.
Communicate Changes: You are responsible for informing your provider of any changes in your condition.
